1. It’s complicated, since the “American Healthcare System” isn’t really a system and isn’t really about healthcare. That insurance is attached to employment is an accident of history dating back to the Second World War.

   Without getting too far into the weeds, it isn’t a bonsai tree, it is a row of hedges planted during the Roosevelt administration that has been permitted to grow wild and mostly untrimmed ever since.
